The price quotation should include the following: 

1) Commercial offer (the template is available in Appendix 1);

2) List and specifications of the devices offered and part numbers, as well as other documents, if any; 

3) MAF/DAF provided by the manufacturer/distributor;

4) Certificates of origin for the items offered (can be provided at the time of purchase).

Please also read the Conflict of Interest document and note that companies and individuals are required to inform Ameriabank about any existing or potential conflict of interest situation prior to procurement. In such cases, the respective bidders will not be allowed to participate in the procurement process unless they disclose the conflict of interest in advance and Ameriabank considers it acceptable.
